For best outcomes and your safety, there is information about you that is required from Genesis Dental Hygiene Care.
Each can be downloaded using the download button to then be printed. You may either scan and email to the office, or bring them with you on your first appointment. There is also the option of not doing any of these, and simply filling them out at the office. Whichever you prefer is what we prefer.
